Thanks for your interest in Underwater Criminal Investigators!

To register for any UCI course, please print a copy of the UCI Application Form and a PADI Medical Statement Form and fill them out carefully. You can mail or email your UCI Application Form along with the Medical Statement to UCI. Keep in mind, any "Yes" answers on the Medical Form will require that you receive a physical examination from a doctor with a statement from him/her stating you are physically able to attend the course.
